Your physical therapist will also communicate with your referring practitioner and other members of your care team, sending initial evaluation notes, follow up notes and a discharge summary at the end of your care period.
Minnesota is a direct access state for physical therapy but often a referral is required from a medical practitioner for insurance coverage. You can check coverage and referral requirement with your insurance provider.
All Medicare patients will need a referral from a primary medical physician in order to see our physical therapist.
